2024 Benchmarking Telehealth Usage in Virginia Survey

Consent for participation in Virtual Care in Virginia: 2024 Benchmarking Survey
As part of ongoing efforts to monitor the impact of telehealth on Virginia’s patients and providers, the Virginia Telehealth Network (VTN) and the Virginia Department of Health (VDH) seek to understand Virginia’s current and future needs with virtual care. 

All actively practicing licensed health care providers in the Commonwealth of Virginia are eligible to participate. Participation in this survey is voluntary and anonymous (unless you elect to provide your email address).

Important notes: Any identifying information you share in your survey response will be kept completely confidential to the full extent of the law. VTN will utilize Typeform as our online survey tool. Typeform does not collect or store IP addresses and instead creates a Network ID which is a “randomly-generated string of characters unique to the IP address of the respondent” that allows for reduced entry duplication. Typeform encrypts data in-transit (end-to-end, including within the virtual private cloud at AWS) using secure TLS cryptographic protocols (TLS 1.2) and Advanced Encryption Standard (AES) is used with a 256-bit key to encrypt data at rest including the backups of the information. 

Study findings will be presented only in summary form. While the investigator(s) will keep any identifying information confidential, there are some risks of data breeches when sending information over the internet that are beyond the control of the investigator(s). Please note: You must be 18 or older to participate in this study.
